Confirmation Process

There are five steps to the Confirmation of Admission process. The process must be completed by the posted deadlines or the offer of admission will be rescinded and you may lose your seat if the course is full. Please refer to Policies and Procedures for specific information. Submission verifies that both student and parent/guardian have read, understand, and agree with each of points below.

  1. Activate Directory ID
  2. Submit Medical Information Form
  3. Submit Group Fitness Program Informed Consent Form
    Form accepted as follows: Print, sign, scan, and email copy to ysp@umd.edu or mail signed form to: Terp Discovery, Suite 0132 Main Administration Building, 7901 Regents Drive, College Park, MD 20742.
  4. Submit Program Acknowledgement Form
    • Student Conduct and Academic Integrity
    • Permission to Leave Campus Policy
    • Meal Plan and Dietary Accommodations Requirements
    • Program Transportation and Liability Waiver
    • Accessibility and Disability Support Services
    • Photography and Videography
  5. Submit Payment
